Please print this form, complete and turn in at Trial I registration.

I/we have received a copy of the 2010-11 Stanislaus County Mock Trial case and rules.  As a condition of participation in this year’s program, I/we agree to discuss these rules with my/our team and to follow all rules and regulations as specified in the California Mock Trial materials or disseminated by Stanislaus County Office of Education (SCOE) staff.  I/we understand that the failure of any member or affiliate of my/our team to adhere to the rules may result in disqualification of the team.

 

I/we understand that this signed teacher-sponsor(s) agreement form, a team ethics form, and a complete set of signed student permission slips (one for each student team member with a parent or guardian’s signature as well as the student’s) must be received by SCOE at the Trial I competition registration.  If these forms are not received, my/our team may be disqualified.
