School Registration Information

School Registration form due no later than November 4, 2011

Registration Instructions: Note the registration fee below and click the
“Register” button to submit registration information and fee total. 

Registration Fee:  
                $450  For one team consisting of 9 students as indicated on the student
                         registration form
               - 350  donated by the Education Foundation of Stanislaus County
                $100    FINAL COST

                           Note:   $12 fee for each additional alternate participant.  An invoice  
                                      will be sent after the event date that will be based on the
                                      number of alternate participants.

The registration fee may be paid by check or purchase order.  Please make checks payable to SCSSF. Purchase order must indicate “Academic Decathlon Registration Fee” and can be faxed to (209) 238-4203, attn: Barbara Little, or send by US or route mail (see below). Print a copy of your registration form before submitting and include it with payment.

You may bring a copy of the Registration Form with payment to the coaches meeting on Thursday, September 14, 4:00-5:30 pm, SCOE, Shiloh Room, 1100 H St., Modesto
